Galaxy Diagnostics is happy to refer healthcare providers to Dr. Bob Mozayeni for a brief phone consultation to discuss test findings. These clinical consultations are coordinated by his private practice, Translational Medicine Group (TMG), for a fee.
*Please note that only treating clinicians of patients who have been tested at Galaxy Diagnostics, Inc. may request a consult. Family members, friends, and patients, even if clinicians, are not eligible.
We encourage you to submit your questions first in writing, providing sufficient detail for Dr. Mozayeni to prepare a response. In the face of significant demand, TMG may first respond in writing, and then, if you wish, arrange for a follow up phone call.
Provider Consult Instructions:
- Submit online request here: https://tmgmd.com/practitioner-consultations/
- Write “Galaxy test results” in the notes field.
- ALL fields must be completed on the practitioner request page, including practitioner name and license number with expiration date.
- After submitting the request, please fax a copy of the patient’s results to TMG at 240-221-0441 with a note referencing your online submission for consultation. Email any questions or background material to medical@tmgmd.com.
