What are Galaxy's main test offerings?

Three platforms anchor the menu: the BBB Direct Detect digital PCR assay (direct, genus-level molecular detection of Bartonella, Borrelia, and Babesia from whole blood, with Bartonella-specific culture enrichment); the Bartonella IgG Detect IFA (indirect serology measuring immune exposure to four clinically relevant Bartonella species in North America); and the Lyme Borrelia Direct Detect Nanotrap urine antigen test (direct detection of OspA). These test offerings are available individually and in bundled 1-day and 3-day panels.

Two-tier serology is indirect: it provides supportive evidence of exposure, not of pathogen presence, and a single EIA or immunoblot alone yields false positives and negatives, which is why two tiers are required. It also underperforms early: only about 30% of acute patients present with an erythema migrans rash, and roughly half of those test serologically negative at that stage. Direct antigen and direct molecular methods can provide evidence before or during seroconversion, complementing the delayed and variable antibody response.

The Bartonella IgG Detect IFA reports individual titers for B. henselae, B. vinsonii berkhoffii, B. koehlerae, and B. quintana, the four species most associated with human infection in North America. Galaxy is one of the few commercial labs to include B. koehlerae, an omission on many other panels that can cause missed exposures.

Typically two to three weeks, three weeks specifically for anything involving the BBB Direct Detect, because of its added week-long culture period. There’s no login needed to place an order; a portal password-setup prompt is sent automatically once the first sample reaches the lab. Results can also be faxed on request, though they’re posted to the portal (Dendi) either way.

A growing body of literature links Bartonella and Babesia to neuropsychiatric and chronic-fatigue presentations, including schizophrenia and adult psychosis case-control work, mild cognitive impairment, pediatric acute-onset neuropsychiatric syndrome, and ME/CFS-consistent illness. The proposed mechanisms include direct endothelial or CNS invasion and immune-mediated inflammation. These associations are why a vector-borne workup can be warranted in otherwise treatment-refractory neuropsychiatric or fatigue syndromes, though causation remains under active study.
