What Personal Information We Collect
Galaxy Diagnostics will collect information that identifies you (“personal information”) online only when you voluntarily provide it to us through our site. For example, when you choose to add your name to our mailing list, we may ask you to provide some personal information, such as your first and last name, mailing address, telephone number and/or e-mail address. In addition, we may collect information that you voluntarily provide to us by email or via an online submission form.
How We Use Personal Information We Collect Online
We may use your personal information within Galaxy Diagnostics (1) to provide you with the services and products you request, (2) to assist with your questions about our services, billing, payment methods or use of our web site, (3) to process or collect payments made in connection with our services to you, and (4) to evaluate our services and products and to conduct customer surveys. We also may use your information within Galaxy Diagnostics, with your consent, to send you information that may be of interest to you. We will not give, sell, rent, loan or otherwise disclose any personal information to any third party, unless (1) you have authorized us to do so, (2) we are legally required to do so, for example, in response to a subpoena, court order or other legal process, and/or (3) it is necessary to do so in order to protect and defend the rights or property of this web site. For example, with your consent, we may disclose your personal information to a third-party vendor that we engage to mail marketing information to you. We contractually require such third-party vendors and contractors to comply with strict standards regarding security and confidentiality. We also may share aggregate, non-personal information about web site usage with unaffiliated third parties. This aggregate information does not contain any personal information about our users.
How We Protect Information Online
Galaxy Diagnostics is committed to protecting your privacy. It is our policy to protect your personal information against unauthorized access or release. All personal data transmissions are encrypted. All the information you provide to us is handled through a Secure Socket Layer (“SSL”), the industry standard for encrypting all personal information, including name, address and credit card information. Please use caution when sending sensitive information via e-mail, as e-mail messages do not have the security features that are built into our website. Except as required by law or as otherwise provided herein, Galaxy Diagnostics will take commercially reasonable steps to ensure your right to privacy. In addition, we have procedures that limit Galaxy Diagnostics’ employees and contractors’ access to personal information. Only those employees and contractors with a business reason to know have access to such information. We educate our employees about the importance of confidentiality and customer privacy through standard operating procedures, mandatory training programs, and internal policies on data privacy and corporate integrity. We will take appropriate disciplinary measures to enforce employee privacy responsibilities.
Patient Access to Laboratory Results
Effective October 6, 2014, the HIPAA regulations changed to allow patients to directly request laboratory results from the laboratory that performed the testing. Galaxy Diagnostics, Inc. has developed a Patient Results Request Form for you to complete and submit. The purpose of the form is to ensure that our laboratory results are released to the patient or legal representative of the patient. In this way, your personal health information is protected. We aim to provide the final laboratory report within seven business days of receipt of the completed form. Please note that medical laboratories are not required to provide clinical interpretation of the results to patients. You are strongly encouraged to discuss your laboratory results with your health care provider.
