Our ELISA and Western Blot tests detect two subclasses of antibodies (IgM and IgG) produced by the immune system in response to Borrelia burgdorferi. IgM antibodies are the first set of antibodies produced by the body approximately 2-4 weeks after B. burgdorferi infection. After approximately 30 days, IgG antibodies are the dominant subclass produced by the body in response to B. burgdorferi infection.

A blood test for antibodies against B. burgdorferi is the recommended test for the diagnosis of Lyme borreliosis (Lyme Disease). However, if a person has central nervous system symptoms, such as meningitis, then IgM, IgG, and western blot testing may sometimes be performed on cerebrospinal fluid.

All lab tests have limitations. Lyme Borreliosis serology is famously limited for a number reasons, related to host response factors and t he way the pathogen behaves in the human body. We offer these CDC-recommended two-tier test options because two-tier testing is t he current “standard of diagnostic care” for confirmation of Lyme Disease and therefore important reference assays for both clinical use and well- designed research studies.

Clinically, the symptoms and risks factors for Lyme borreliosis are very similar to other tick-borne infection, most notably bartonellosis and babesiosis. Furthermore, these vector-borne diseases can mimic a number of immune mediated disorders, leading to potential mis diagnosis. Given clinical concerns about misdiagnosis and potential cross-reactivity of antibodies for bartonellosis and other infections, we are expanding our menu to better allow clinicians to test for evidence of potential co-infections in patient specimens.
