Step 1 – Order a Specimen Collection Kit
All test orders must be ordered by a licensed medical practitioner. The Specimen Collection Kit Order Form is available on our website. Be sure to fill out the form and email or fax it to us. Once we receive your order form, we will mail you the kit.
Step 2 – Collect the Sample
The specimen kit will include detailed instructions on sample collection and shipping, blood tubes, all required test forms, and shipping materials. For sample types other than blood, please review our Sample Instructions for required quantities and containers. Call us at 919-313-9672 if you have questions.
Step 3 – Pack and Ship the Sample Kit
Pack the sample along with the required forms and ice pack (included in the kit) and ship overnight to our physical address in Research Triangle Park, NC. DO NOT USE COURIER DROP BOXES. Specimens are stable at room temperature for up to two days, but must be protected from high heat. See our Shipping Instructions for more information and for international shipping requirements.
Step 4 – Send Shipment Notification
Once your package is ready for shipment please send us a shipment notification by email, fax, or phone. This notification will allow us to troubleshoot any potential problems asap.
